By 2016, only 7% of Fairfield County adults will be without health insurance.

· Support programs that lead to healthy, active lifestyles

· Reduce stigma associated with receiving professional help
· Support healthy environments (personal and communal)

Arthritis Foundation
• 84% reported reduced joint pain
• 78% reported improved muscle strength
• 91% engaged in 120 minutes of exercise each week or more

By The Way Medical Mission Free Clinic
• 100% of people seeking care, received care.

FairHoPe Hospice Grief Services
• 100% of participants maintained or increased their understanding of and better able to deal with grief issues
• 75% of participants identified as high risk on the Bereavement Risk Assessment decreased their level of risk as a result of the program
• 99% of PALS (Children's Grief Support) participants demonstrated an "open" socialization communication style at the conclusion of the program.

New Horizons - TeenScreen
• 100% of tested positive students received same-day in school mental health assessment and parental consultation
• 62% of tested positive students were referred for other services
• 51% of those referred accepted the referral
• 24% were confirmed to have attended first appointment
• 55% of parents returning permission slips granted permission for participation in the screening
